Before your first visit to our office, you may want to review the policy and complete the two forms, New Patient Registration and the Acknowledgement of the Privacy Policy. Bringing these completed forms with you will save some time on your first visit. If you are not able to complete them in advance, we will be happy to give you copies to complete when you come to our office.
